We are not in network for Medicare, Medicaid, Saint Alphonsus Health Alliance, IDID or Saint Luke’s Health Partners. For the plans we do offer we recommend that you contact your insurance company to verify coverage for in-network visits. You can find this out by calling the number on the back of your card or logging into your insurance account and using a ‘Find a Provider’ tool.
